Why are your hormones attacking your hair? And most importantly, is there anything you can do to stop it?

The answer is yes, there is a lot that can be done.

But first you need to understand exactly what's happening inside your body and why hormones have so much power over your hair.

What are hormones and why do they control your hair?

Hormones are chemical substances produced by special glands in your body that function as messengers.

They travel through the bloodstream carrying instructions to different organs and tissues on how they should function.

Think of them as bosses who send messages telling the cells what to do: grow, stop growing, produce something, stop producing.

Hair follicles, which are the structures in your skin from which hair strands grow, have special receptors for hormones.

This means they are extremely sensitive to hormonal messages and respond directly to changes in hormone levels.

For women, the most important hormones related to hair are estrogen, progesterone, and androgens.

Estrogen is considered the hair-friendly hormone.

When estrogen levels are good, it prolongs the hair growth phase, keeps more hair on the head, and makes the hair thicker and healthier.

That's why pregnant women, who have very high estrogen levels, usually have beautiful, voluminous hair during pregnancy.

Progesterone also plays a protective role, although less pronounced than estrogen.

Androgens are hormones that include testosterone and its derivatives, such as DHT.

Although they are called male hormones, all women produce androgens in small quantities through the ovaries and adrenal glands.

At normal and balanced levels, androgens do not cause problems.

But when levels are elevated, or when the ratio between androgens and estrogen becomes unbalanced, they can have a negative effect on hair.

Here's the crucial part you need to understand: androgen sensitivity is genetically determined.

Some women are born with hair follicles that have many receptors for androgens and are very sensitive to these hormones.

When androgens interact with these sensitive follicles, they trigger a process called miniaturization.

With each new growth cycle, the follicle produces a hair strand that is slightly thinner and shorter than the previous one.

Over the years, the hair strands become progressively thinner, almost like baby down.

In advanced cases, the follicles may eventually stop producing visible hair, although they are still technically alive.

This process is androgenetic alopecia, also known as female pattern baldness or female pattern baldness.

It is the most common cause of hormone-related hair loss in women.

The Main Causes of Hormonal Imbalance

Several different scenarios can cause hormonal imbalances that affect hair, and understanding which one applies to you is crucial for choosing the right treatment.

Polycystic Ovary Syndrome (PCOS)

PCOS is one of the most common hormonal causes of hair loss in women of reproductive age.

In this condition, the ovaries produce excess androgens, primarily testosterone.

This excess of androgens interferes with normal ovulation, causing irregular or absent menstrual cycles.

Women with PCOS often have a combination of symptoms: irregular cycles, difficulty getting pregnant, weight gain, especially in the abdomen, insulin resistance, acne, male-pattern hair growth (face, chest, abdomen), and thinning hair on the scalp.

The pattern of hair loss is generally diffuse thinning on the top of the head, while the frontal hairline remains relatively preserved.

PCOS affects about 10 percent of women of reproductive age and often goes undiagnosed for years.

Many women only discover this when they begin to investigate menstrual irregularities or difficulty getting pregnant.

Thyroid Dysfunctions

The thyroid is a butterfly-shaped gland located in the neck that produces hormones that regulate the body's metabolism.

When the thyroid gland works too hard, producing excess hormones, we call it hyperthyroidism.

When it is underactive, producing insufficient hormones, we call it hypothyroidism.

Both conditions can cause diffuse hair loss because thyroid hormones are essential for the proper functioning of hair follicles.

In hypothyroidism, which is more common, hair loss occurs diffusely, the strands become dry, brittle, dull, and grow very slowly.

Other symptoms include extreme fatigue, weight gain, constant feeling of cold, dry skin, constipation, and depression.

In hyperthyroidism, hair loss is also diffuse, but the strands can become very thin and delicate.

Associated symptoms include weight loss despite eating normally, nervousness, anxiety, palpitations, feeling hot, and tremors.

Thyroid problems are very common in women, especially after the age of 30, and are often not diagnosed because the symptoms are vague and attributed to other causes.

Menopause and Perimenopause

Menopause is the time when the ovaries stop functioning and producing estrogen and progesterone.

Technically, it is diagnosed after 12 consecutive months without menstruation.

The average age for menopause is around 51 years, but it can occur naturally between 45 and 55 years of age.

Perimenopause is the transition period preceding menopause, which can last for several years, when hormones begin to fluctuate unpredictably.

During this period, estrogen levels gradually decrease and eventually plummet.

With less estrogen to protect and balance, the androgens that continue to be produced by the adrenal glands end up having a proportionally greater influence.

Even if absolute androgen levels do not increase, the ratio between androgens and estrogen changes drastically.

In genetically predisposed women, this hormonal change can reveal or accelerate androgenetic alopecia.

Many women who have never had hair problems begin to notice significant thinning after menopause.

The hair strands become thinner, grow more slowly, the growth phase shortens, and the overall volume visibly decreases.

Postpartum

Although we've already discussed postpartum hair loss in another article, it's worth mentioning here because it's hormonal.

The drop in hormone levels that occurs after having a baby is caused by the sharp decline in estrogen levels following childbirth.

This is usually temporary and resolves itself, unlike the other hormonal causes we mentioned.

Use or Discontinuation of Hormonal Contraceptives

Starting or stopping birth control pills represents a hormonal change that can affect hair.

Some birth control pills, especially those with progestins that have more androgenic activity, can worsen hair loss in predisposed women.

Other pills, with antiandrogenic progestogens, may improve hair loss.

Stopping the pill after years of use can trigger temporary hair loss due to hormonal readjustment.

How to Tell if Your Hair Loss Is Hormonal

Identifying whether hair loss is hormonal involves connecting the dots between various signs and symptoms.

If you have hair loss AND irregular menstrual cycles, adult acne, unwanted hair growth, and unexplained weight gain, this strongly points to a hormonal cause, possibly PCOS.

If you experience hair loss and extreme fatigue, weight changes, or abnormal sensitivity to cold or heat, this suggests a thyroid problem.

If you are in the 45-55 age range, your cycles are becoming irregular or have stopped, and you've experienced hair thinning, menopause is the likely cause.

If you have a strong family history of baldness, either in women or men in your family, and you are experiencing progressive thinning, especially on the top of your head, androgenetic alopecia is very likely.

The only way to confirm is through a medical evaluation with blood tests.

Important hormone tests include total and free testosterone, DHEA-S, androstenedione (to assess androgens), TSH, T3 and T4 (for thyroid), FSH and LH (to assess ovarian function and menopause).

It is often also helpful to perform an ultrasound of the ovaries to check for the presence of multiple cysts characteristic of PCOS.

A dermatologist or endocrinologist can order these tests and interpret the results in the context of your symptoms.

Treatment Options for Hormonal Decline

The good news is that there are effective treatments for hormonal hair loss, but the approach varies depending on the specific cause.

For SOP

Treatment for PCOS typically involves oral contraceptives with antiandrogenic properties.

Pills containing progestogens such as cyproterone, drospirenone, or dienogest help to reduce androgen levels and regulate cycles.

Spironolactone, a medication that blocks the action of androgens in tissues, is often prescribed along with birth control pills to further improve hair.

Metformin, a medication for insulin resistance, can help with both metabolic symptoms and hair loss.

Lifestyle changes, including weight loss if overweight, regular exercise, and a diet with controlled carbohydrates, significantly improve PCOS and consequently the hair.

For Thyroid Problems

Treatment for hypothyroidism involves hormone replacement therapy, usually with levothyroxine.

Once thyroid hormone levels normalize, the decline usually stops and growth returns to normal.

This can take a few months because there is a delay between hormonal normalization and hair growth response.

For hyperthyroidism, treatment may involve antithyroid medications, radioactive iodine, or surgery, depending on the cause.

For Menopause

Hormone replacement therapy (HRT) can help alleviate hair thinning during menopause.

Systemic or local estrogen helps to balance the ratio with androgens.

But not all women are candidates for HRT, as there are contraindications and risks that need to be assessed individually.

Topical minoxidil, sold in pharmacies, is a proven treatment for androgenetic alopecia and can be used during menopause.

It stimulates hair growth and is applied directly to the scalp.

For Androgenetic Alopecia in General

Minoxidil is the most established topical treatment, available in 2 and 5 percent concentrations.

It works best when started early, before the thinning becomes too severe.

Oral finasteride, a medication approved for male pattern baldness, is sometimes used off-label in postmenopausal women, but it is contraindicated in women of reproductive age.

Oral spironolactone blocks androgens and may improve hair, especially when combined with birth control pills.

Dutasteride, similar to finasteride but more potent, can also be used in selected cases.

Treatments such as scalp microneedling, low-level laser therapy, and applications of stimulating peptides are complementary options.

Hair transplantation is possible in women, but it is generally only considered when medical treatments have failed and suitable donor areas are available.

What You Can Do Now

If you suspect your hair loss is hormonal, the first step is to seek proper medical evaluation.

A dermatologist specializing in hair or an endocrinologist can properly investigate this.

Don't wait for the situation to get much worse before seeking help, because treatments work best when started early.

While waiting for your appointment or test results, you can start taking better care of your overall health in ways that help balance your hormones.

Maintain a healthy weight through a balanced diet and regular exercise, because excess body fat worsens hormonal imbalances.

Get enough sleep, because inadequate sleep messes with your hormones.

Manage stress using techniques that work for you, because chronic stress raises cortisol levels, which interferes with other hormones.

Avoid fad diets or overly restrictive diets that can worsen imbalances.

Be gentle with your hair while it's vulnerable, avoiding harsh chemicals and excessive heat.

The Truth About Hormonal Decline

Here's what you need to understand: hormonal hair loss is usually progressive if left untreated.

Unlike hair loss due to stress or nutritional deficiencies, which stop when the cause is corrected, hormonal hair loss tends to continue and worsen over time.

Androgenetic alopecia, in particular, is a chronic condition that, without treatment, results in progressively thinning hair.

But it's also true that treatments exist and work, especially when started early.

You don't have to passively accept losing your hair.

Investigating, correctly diagnosing, and treating appropriately can preserve much of the hair you still have and, in many cases, restore lost density.

Your hormones are not the enemy, they are messengers.

And when they are out of balance, your whole body suffers, not just your hair.

Addressing hormonal imbalances improves not only your hair, but also your energy, mood, weight, and overall health.

You deserve to feel good about your body and confident in your appearance.

And your hair deserves the chance to grow healthy in a hormonally balanced body.

Hormonal imbalances can damage your hair,
